Heart & Soul

A collaborative effort among TCDC, the Black Nurses Association - Greater Phoenix Area and St. Luke’s Health Initiatives to reduce heart and lung disease among African-Americans and other racial/ethnic minorities. This initiative targets the homeless, teens and their parents, the faith-based and broader community and focuses on healthy eating, physical activity, cardiovascular and lung disease education, blood pressure screening, tobacco awareness and prevention. Initiatives include a Food Pantry, a Youth & Adult Cardiovascular Programs, Faith & Community Collaborative Summits, and Community Gardens, and Asset Based Communities of Practice. It’s all based on the assets, strengths, passions and gifts of the community.

The Heart and Soul AZ Challenge is an easy way for friends, family, and co-workers to break through the barriers in pursuing a healthy future. Through a six week program, individuals will learn about the importance and benefits of nutrition and exercise while being rewarded for staying active.
